Bury South Older People’s Community Mental Health Team (CMHT)
-
The Bury South Older People’s Community Mental Health Team (CMHT) provides specialist mental health assessment, treatment and support for older adults (aged 75 and over) in the Bury South area of West Suffolk. The team works with people experiencing a range of mental health conditions associated with ageing, including dementia, depression, anxiety, psychosis, and other complex mental health needs. We also support people under 65 who have a diagnosis of dementia or an age-related mental health condition where their needs are best met by an older people’s service.

Access this service
Referrals are accepted from GPs and other healthcare professionals via the Access and Assessment Service in Suffolk. Following referral, the team will make contact to discuss your needs and arrange an initial assessment.
Assessments may take place in clinic or at home depending on clinical need. Once assessed, a care plan is agreed with you which may include medication, referral to psychology or occupational therapy, support from a care coordinator, or signposting to other relevant services. The team also works closely with the Dementia Intensive Support Team (DIST) and the Crisis Resolution and Home Treatment Team (CRHTT) to ensure continuity of care across services.

How this service support carers including friends and family
We include carers, friends and family in care planning wherever appropriate. The team can refer carers for carer assessments and direct them to community support services.
We encourage nearest relatives and next of kin to take part in decisions about care and invite them to formal reviews as part of the Care Programme Approach. We can provide advice and signposting on topics such as Lasting Power of Attorney, attendance allowance, and local carer support organisations.
