One-page profiles

One-page profiles are a simple summary of the things that you would like other people to know about you. They can help to make sure your voice is heard so that other people know how you want to be supported. 

One-page profiles include:

  • What's important to you?
  • Your strengths or the things other people admire about you
  • How best to support you

You can add other information to one-page profiles. When this happens, the document may have a different name such as student passport or an individual support passport. The name doesn’t matter. What is important is that the information you want to share with people is included. 

These may include:

  • What you like to be called
  • What you are good at and what is more difficult for you
  • How to best communicate with you
  • How you ask for help and what you might look like if you’re unable to
  • Health information
  • Information and contact details for other people that support you
  • Any reasonable adjustments that you need.  A reasonable adjustment is when a service, for example a school, college or workplace changes the way they usually do things.  This is to help make sure that everyone can use their services.

Why one-page profiles or support passports are useful:

  • They tell people about you and what is important to you
  • They can help people to get to know you 
  • They ensure you have a say in how you want to be supported  
  • They make sure the people that support you know how you want to be supported

How to use your one-page profile or support passport:

One-page profiles or support passports can be useful when you are meeting new people or going to new places (such as starting a new social club, training course or job). You can use your one-page profile or support passport in different ways. You might:

  • Send a copy to people before you meet them. 
  • Take a copy to your first meeting. You might want to look at this together and talk about the things that are important to you or any changes that might be helpful in the new environment.

It’s important to keep a one-page profile or support passport up to date, so that other people know if the things that help you have changed.

Lots of different people use one page profiles or support passports to share information about themselves and how they would like to be supported.
