 What help do you offer?

We offer support if you are an adult experiencing memory problem’s or with severe mental health problems and complex physical frailties who need assessment and support in the community setting.

 How do I get help from your service?

Please visit your GP who can arrange for appropriate investigations, such as blood tests and ECG tests if appropriate, and then they will refer you if necessary, to the service through our single point of advice line on 0300 7900371.

Our dementia intensive support team (DIST) are available 8am until 8pm, seven days a week (including Bank Holidays) for people in crisis and needing more intensive support during that time.

 What can I expect from you?

We offer assessments for both memory and functional mental health problems in our assessment clinics or in the home environment (if there is a clinical need).

These are carried out by Consultant Psychiatrist, experienced Non-Medical Prescribers and Mental Health Nurses to enable diagnosis and treatment plans where appropriate. In some cases where further input is not required, the team can offer advice and signposting to other relevant agencies.

The community team hold regular Multi-Disciplinary Team meetings (MDT) where your case is discussed and reviewed with the Consultant and other relevant team members ensuring appropriate support, advice, guidance and signposting to other available services in your local area is offered.

 How can I contact you?

Once you are referred to us by your GP and assessed our contact details are 01553 609933 (option 2). Your clinician gives you a dedicated nighttime number to use after 5pm.

In office hours, please contact your care coordinator, lead professional or ask for a duty worker. Out of office hours, you can contact your out of hours GP service or in an emergency 111 service or 999.

 What support do you offer carers including friends and family?

We offer assessment, support, advice, education, psychological therapy with the signposting and guidance, on an individual basis, as appropriate. This can be done either in clinic or home environment.
