Once we receive your referral, we organise a time and date to meet you. At this session we assess you. From this assessment we agree a plan of action with you. This could be advice and signposting or you could be taken on by the community team for specific interventions to help with your mental health issue.
If you have been referred for an assessment of your memory, we either see you at your home address or invite you to attend a clinic appointment.
These interventions could include:
- Assessment and diagnosis of your mental health problems
- Assessment and treatment of memory problems
- Risk assessment and risk management
- Monitoring of mental health difficulties and your agreed treatment plans
- Therapeutic interventions including individual and group work
- Specialist psychology input
If you would benefit clinically, we also put you on a trial of memory medication.
