Integrated Delivery Team (IDT) Ipswich

 What help do you offer?

​In the IDT, we are responsible for the coordinated delivery of community mental health services in our designated locality. This includes support for children and their families, adolescents, working age and older adults, and individuals with a learning disability.

We provide clinical interventions to address complex mental health needs, as well as certain specialist difficulties such as ADHD and autism. We have a range of qualified and experienced clinicians including doctors, nurses, social workers, occupational therapists, support workers, psychologists and psychological therapists.

 How do I get help from your service?

Most of our referrals come from GPs, following prescribing and other interventions within their guidance, through to the Access and Assessment team. Other professional agencies can also refer you.

Our Access and Assessment teams provide you a standalone comprehensive assessment from qualified and experienced clinicians, who explore a range of your health and social care needs. Subsequently you could be signposted to universal mainstream resources or else referred into the IDT.

In the IDT we allocate a care coordinator to arrange your care package and establish a care plan with you. We review and adjust your care plan regularly. This is known as the Care Programme Approach (CPA).

 What hours is your service available?



The IDT links with a number of other service areas, such as the Access and Assessment team, and Home Treatment team, to provide a 24 hour assessment and intervention service.

 What can I expect from you?

​If you are receiving a care package under the Care Programme Approach, you have your care coordinator as your primary link in the IDT, but you may also talk to other clinicians and administrators as part of your care plan. You will know how to get hold of your care coordinator if you need to, including during times of urgency.

You have an arranged schedule of contact. For some people this is intensive, involving face-to-face or telephone contact several times a week. For others this involves a monitoring service with weekly or monthly contact. Some contact occurs individually in a clinic, at your home or in the community. At other times we deliver evidence-based interventions in groups.

 What support do you offer carers including friends and family?

​We include your friends and family in care planning and perform carer assessments to reflect their role as significant contributors to the care plan. We encourage your nearest relatives and next of kin to participate in decisions made with you about your care, and invite them to formal reviews as well as supporting the interventions described as part of the Care Programme Approach.
