Our Access and Assessment Team comprises of
a team of Mental Health Practitioners and administrators and offer screenings
and assessments for people with a severe and enduring mental illness. We are able to signpost referrals to
specialist service lines within the Trust.
General Practitioners and healthcare professionals can refer into our
service electronically via E-Referral, email or telephone. We offer a 24 hour service for referrals that
are requested on an emergency basis.
If you are not currently under the care of
Mental Health Services, any professional involved in your healthcare can make a
referral to the Access Team. Once a
referral has been received into our service a Mental Health Practitioner will
make a clinical decision as to the right pathway, to offer an assessment, signpost
to Primary Care or refer onto specialist services.
Our service is available 24 hours a day
for an emergency response time via your General Practitioner or Healthcare
Our office hours for referrals to be made on a routine or urgent basis are 9:00 to 17:00, Monday to Friday.
We will offer an assessment via telephone,
video call or face to face on a routine, urgent or emergency basis. On completion of an assessment we will ensure
that further mental health support is offered if felt appropriate, signpost to
external services or discharge you back to the care of your General

We provide advice and assessment on all
mental health conditions for ages 26 and above.
We can offer advice and information to
family cares on general matters and, with your consent, can provide specific
advice and support on your individual case.

If you already have a named Mental Health Professional assigned to
support your mental health needs you would be advised to contact them in the
first instance. If you do not have a
Mental Health Professional or it is a medical emergency then contact your GP
who can generate a referral to the Access Team on an emergency, urgent or
Onsite facilitiesWe have no parking facilities for our Waveney patients however local authority parking is available nearby.
Free parking is available for our Great Yarmouth patients onsite and just outside of the hospital.
We have full disabled access and toilet facilities.
