Dementia and Complexity in Later Life South Central Norfolk

 What help do you offer?

We are the DCLL CMHT for South Norfolk, responsible for assessment of patients referred within this group and their treatment. Following assessment by the most appropriate clinician within the team a treatment plan will be discussed with you and your carers We also offer some groups and signposting on to other organisations. We offer a memory medication service for patients with dementia.

 How do I get help from your service?

​Referral via your Gp or other healthcare professional, for example district nurse or Community OT

 What hours is your service available?

Monday to Friday office hours, no bank holiday cover.

 What can I expect from you?

You will receive a letter or phone call regarding an appointment either at your house or one of our clinics.  Following assessment a treatment plan will be discussed and implemented.  You can request a copy of all correspondence and will be offered a copy of all care plans.     ​

 What is the main address of your service?

​The team are based at Gateway House in Wymondham and run clinics at Attleborough (Bickley day treatment centre) and The Julian Hospital in Norwich

 How can I contact you?

​Phone 01953 611002 for all queries, we have a duty worker every day who can take queries if your CMHN isn't available

 What conditions do you treat?

​All age dementias, and patients with functional illnesses such as depression and anxiety aged over 65 with age related physical health problems which may impact on their mental health.

 What support do you offer carers including friends and family?

​We offer advice, individual support and information for carers. We also run carer support groups.
