Dementia and Complexity in Later Life South Central Norfolk

 What help do you offer?

We offer assessments and interventions if you have age-related needs and are suffering from acute, severe and enduring mental health problems including anxiety, depression, psychosis, confusion, dementia and related behavioural problems. Our services are not exclusively for people over 65.

Our team works alongside other teams and organisations to provide interventions.
Our specific functions include:

  • Assessment and diagnosis of your mental health problems
  • Assessment and treatment of memory problems
  • Risk assessment and risk management
  • Monitoring of mental health difficulties and your agreed treatment plans
  • Therapeutic interventions including individual and group work
  • Specialist psychology input

 

 How do I get help from your service?

We accept all referrals through the Single Point of Access team. To self-refer into our service, please call 0300 123 1503. For general enquires you can also email: wellbeing.centralnorfolk@nsft.nhs.uk.

 What hours is your service available?

Monday09:00 to 17:00
Tuesday 09:00 to 17:00
Wednesday 09:00 to 17:00
Thursday 09:00 to 17:00
Friday 09:00 to 17:00

 What can I expect from you?

Once we receive your referral, we organise a time and date to meet you. At this session we assess you. From this assessment we agree a plan of action with you. This could be advice and signposting or you could be taken on by the community team for specific interventions to help with your mental health issue.


If you have been referred for an assessment of your memory, we either see you at your home address or invite you to attend a clinic appointment.

These interventions could include:

  • Assessment and diagnosis of your mental health problems
  • Assessment and treatment of memory problems
  • Risk assessment and risk management
  • Monitoring of mental health difficulties and your agreed treatment plans
  • Therapeutic interventions including individual and group work
  • Specialist psychology input

 

If you would benefit clinically, we also put you on a trial of memory medication.

​We offer advice, individual support and information for carers. We also run carer support groups.
